How to Register as a New Patient

Step 1 – complete new patient forms as below
The forms you need to complete depend on how many people are registering together.

If you need the forms in a different format or need help to complete them - please let us know.
Please note that if you take regular medication - you will need to make sure you have enough medication from your last surgery to last for a month before you register with us.
Step 2 – bring your completed forms and proof of your address to the practice
We accept new patient registrations anytime between 10am and 5pm (Monday to Friday).
In order to register with our practice, you need to come to the practice and bring one complete set of the above forms for every person who wishes to register with us.
You also need to bring:
- photographic ID – an official document that has your photo and name on it such as a passport or a photo ID driving licence
- proof of your address – a recent document (less than 3 months old) that proves where you live, such as a gas or electricity bill, or a bank statement.
We will go through your forms and documentation with you - so that we can make sure that we have everything that we need to register you. We will also let you know who your 'named accountable GP is':
What is your 'named accountable GP':
Your 'named GP' is the GP who will ensure that we work with other health and care providers to deliver a health and care package that meets your needs.
Things to know about your 'named GP':
- You can consult with any of our GPs or other clinicians - not just your 'named GP'
- Your 'named GP' can't be available all of the time (they have families too!) - so other GPs and clinicians will support you too
- If you would like to have a different 'named GP' to the one that we allocate to you - just let us know and we will try to meet your preference. If this is not possible - we will let tell you that we can't and will explain the reasons why
- Your 'named GP' is not legally responsible for the support you receive from other health and care professionals
Step 3 – we complete the registration process
We will complete the registration process and add you to our computer systems. We also contact an organisation called the ‘Primary Care Support Service’ who will make sure that your medical records from your previous GP are transferred to us.
Changes to personal details
Please make sure we have an up to date record of your personal details. If you change your name, address or telephone number please inform reception. We may need to contact you urgently on occasion.
